Share the Care Dental


	Who we accept referrals from. What are the qualifications. What to do if the child qualifies. What to do if the child doesn't qualify.

	We accept referrals from:
		- school nurse and health assistant - health clinics - community agencies back to top
	Qualifying children are:
		- in urgent need of dental care (pain or infection), - age 5-18 years, - receiving or eligible for free or reduced school lunch, - without resources for dental care (this means no insurance, including Denti-Cal) back to top
	If the child qualifies:
		1. Call Share the Care at 619-692-8858, Monday - Friday 8:30am-4:00pm Have ready: -child's name, birthdate, dental problem -parent/guardian name, address (zip code necessary), phone number, and language spoken -availability of transportation -need for translator -your name, school/agency/organization, phone number, and fax number 2. STC will then locate a dentist and make an appointment after speaking with the parent or guardian. -Referral source will be notified of the appointment. -Dentist will make any subsequent appointments directly with family. Other important information: -Child must be accompanied by parent/guardian. -A"no-show", cancellation, or arriving late can result in no assistance from the program. Please help educate the parent/guardian about the importance of keeping the appointment and arriving promptly. -Family is responsible for purchase of any needed prescriptions. back to top
	What happens if a child doesn't qualify?
		-Contact Share the Care at 619-692-8858 for a list of low-cost dental care resources. -Have client call Child Health and Disability Prevention (CHDP) Program for assistance in locating low-cost dental and medical care 619-692-8428 (North County: 1-800-675-2229) -Have client call San Diego Kids Health Assurance Network (SD-KHAN) at 1-800-675-2229 for referrals to low-cost dental and medical insurance. back to top
